E-Sign Consent

Mejor Vida Insurance LLC is a licensed insurance agency specializing in final expense life insurance. This page describes your consent to conduct business electronically with our agency. Nebraska Department of Insurance producer license #21695431.

Consent to Electronic Transactions

By providing your electronic signature (by clicking a button such as Continue, checking a box, or otherwise affirmatively indicating your agreement on our website or with our agents), you voluntarily consent to conduct business electronically with Mejor Vida Insurance LLC (“we,” “us,” or “our”) and to receive and sign documents electronically (“E-Sign”).

Documents covered by this consent include, but are not limited to: insurance applications, authorizations, acknowledgments, disclosures, and other documents related to insurance products offered through our agency (collectively, “Documents”). You agree that your electronic signature has the same legal effect as a handwritten (“wet”) signature.

Scope of Consent

This consent applies to Documents presented to you in connection with your insurance inquiry, application, or policy through Mejor Vida Insurance LLC and insurance carriers we work with on your behalf.

When we make a Document available to you electronically and notify you by email or other electronic means, that has the same meaning and effect as if we provided a paper copy, whether or not you choose to view it.

Carrier-specific forms: When you apply for coverage with a particular insurance carrier, that carrier may require its own separate authorizations and disclosures. Those carrier documents are provided at the application stage and are in addition to this agency E-Sign consent.

Required Hardware and Software

To access, receive, and retain electronic Documents, you will need:

• A current version of a supported internet browser (such as Chrome, Safari, Firefox, or Edge)
• An active internet connection
• A program or browser capable of reading and displaying PDF files
• An active email account
• A computer, smartphone, or tablet capable of supporting the above

You are responsible for ensuring your device meets these requirements. Please notify us if your contact information or email address changes so we can continue to deliver Documents to you.

How to Obtain Paper Copies

You have the right to receive paper copies of any Document. To request a paper copy, contact us at Julie@mejorvidainsurance.com or 402-440-5438. Include your full name, mailing address, and phone number. We do not charge a fee for paper copies unless required by applicable law.

How to Withdraw Your Consent

You may withdraw your consent to E-Sign at any time by emailing Julie@mejorvidainsurance.com with the subject line “Withdraw E-Sign Consent” and including your full name, mailing address, and phone number. Please state: “I do not wish to sign Documents electronically.”

Withdrawal of your consent will be effective shortly after we receive it. Please note:

• Withdrawing consent does not revoke any electronic signatures you have already provided.
• Withdrawing consent may affect your ability to complete an insurance application through our agency, as some processes require electronic signatures.
• There are no fees associated with withdrawing your E-Sign consent.

Keeping Your Contact Information Current

You are responsible for keeping your email address and contact information current with us. If we send a Document to your email address on file and it is returned as undeliverable, we will be deemed to have provided that communication to you.

By continuing through our website or affirmatively agreeing with our agents, you acknowledge that:

• You have read and understand this E-Sign Consent.
• Your device meets the hardware and software requirements described above.
• You consent to conduct business electronically and to sign Documents electronically where applicable.
• You understand how to withdraw your consent and request paper copies.
